Foundayo starts at 0.8 mg once daily and climbs through six strengths to a maximum of 17.2 mg, with at least 30 days at every step. That makes the earliest possible arrival at the top dose around day 150 — five months of climbing before you are on the dose the trial results describe. Two rules can send you back down, and one of them is easy to trigger.

The label sets 30 days as a minimum, not a schedule. From 5.5 mg upward it says increases are made "based on treatment response and tolerability", which means a clinician can hold you at a dose that is working, or hold you longer at one you are struggling with. Nobody is obliged to reach 17.2 mg.
The escalation exists to reduce gastrointestinal reactions, and the label is explicit that nausea, vomiting and diarrhoea are most frequent during it. The full side-effect table by dose shows how that plays out.
How to take it
- Once daily, with or without food. No fasting window, no water restriction, no waiting before breakfast.
- Swallow whole. Do not break, crush or chew.
- Never more than one tablet a day, whatever the strength.

The seven-day rule
Miss one dose and the instruction is straightforward: take it as soon as you can, and do not double up the next day.
Miss seven or more consecutive doses and you restart the escalation at a lower dose. Not the dose you were on — a lower one, to avoid re-entering at a level your stomach is no longer used to.
A week is easy to lose. A holiday where the bottle stayed home, a prescription that did not get refilled in time, an illness. On a weekly injection missing a week means missing one dose; on a daily tablet it means missing seven, and the cost is roughly a month of climbing back. It is worth planning refills around.
The interaction the injectables do not have
This is the part of Foundayo's dosing that has no equivalent in Ozempic, Wegovy, Mounjaro or Zepbound, and it follows directly from what makes the drug a tablet.
Semaglutide and tirzepatide are peptides. They are broken down by general protein-degrading enzymes and do not compete for the liver's drug-metabolising machinery. Foundayo is a conventional small molecule, and it goes through CYP3A4 — the same enzyme that handles a large share of prescription drugs.
- Strong CYP3A4 inhibitors: the maximum Foundayo dose drops from 17.2 mg to 9 mg daily. Strong inhibitors that also block OATP1B should be avoided entirely.
- Strong CYP3A4 inducers: avoid. They can lower Foundayo exposure enough to matter.
- Moderate inducers: allowed, but the label says to monitor whether Foundayo is still working and escalate the dose if needed.
Two more from the interactions section. Simvastatin is capped at 20 mg daily alongside Foundayo, because its active metabolite roughly doubles. And oral contraceptives need a backup method for 30 days after starting and after each dose increase, because Foundayo delays gastric emptying — six separate windows across the climb.
None of this makes the drug harder to take. It makes the medication list matter in a way it does not with an injectable, which is a question for whoever writes the prescription rather than something to work out yourself.
Which tablet is which
Six strengths, three colours, two shapes. The lower three are round; the upper three are modified ovals debossed "Lilly" rather than "L".

The colours repeat across the two shapes, so a pink tablet is either 0.8 mg or 9 mg depending on whether it is round or oval — an eleven-fold difference. The G-number on the reverse is the reliable identifier.
Who should not take it
Foundayo is contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2, and with known serious hypersensitivity to orforglipron. It is not recommended in severe hepatic impairment, and should be stopped when a pregnancy is recognised.
