Wegovy comes as two different pens, and they are not used the same way. Before anything else, work out which one you are holding.

If you have been on the single-dose pen and are handed the FlexTouch, the labelling says plainly to get familiar with it first. It is a different device that happens to contain the same drug.
The rule most likely to catch you out
The FlexTouch is flow-checked only before your first injection from each new pen. The Instructions for Use states it twice: "Only check the medicine flow before your first injection with each new pen."
That matters because the Zepbound KwikPen — the same four-dose format, the same drug class — must be primed before every single weekly injection. Two multi-dose pens, opposite rules, and nothing on either device tells you which convention applies.

The single-dose pen
The simpler device, and the one most people start on.
What you need
- The Wegovy pen
- An alcohol swab, or soap and water
- A gauze pad or cotton ball
- A sharps disposal container
No separate needle — it is built in and stays hidden throughout.
Step 1: check the pen
Wash your hands, then do not use the pen if it appears used, if any part looks broken (for example after a drop), if the medicine is not clear and colourless through the window, or if the expiry date has passed. The IFU gives a number to call if any of those checks fail.
Step 2: choose and clean the site
- Upper leg (front of the thighs), lower stomach at least 2 inches from the belly button, or upper arm.
- Avoid skin that is tender, bruised, red or hard, and areas with scars or stretch marks.
- You may use the same body area each week, but not the same spot.
- Clean with an alcohol swab or soap and water, let it dry, and do not touch the site afterwards.
Step 3: inject, and watch the yellow bar
This is where a full dose is won or lost. The injection takes about 5 to 10 seconds, and the yellow bar is the only signal that it has finished.
The IFU is specific about how it goes wrong: you will not get your full dose if the pen is removed too early, or if you have not pressed it firmly against the skin for the entire injection.
So keep pressing until the yellow bar has stopped moving, then lift the pen slowly.
And the part that makes early removal unrecoverable: "The needle cover will lock when the pen is removed from your skin. You cannot stop the injection and restart it later." There is no second attempt with that pen.
If the yellow bar does not start moving, or stops mid-injection, the IFU says to contact your provider or the manufacturer rather than to try again.
Afterwards
If blood appears, press the site lightly with gauze or a cotton ball.
The FlexTouch four-dose pen
Holds four fixed 2.4 mg doses, one taken weekly, and takes separate needles.
The flow check, first injection only
- Turn the dose selector until the counter shows the flow check symbol, lined up with the dose pointer.
- Hold the pen with the needle pointing up.
- Press and hold the dose button until the counter shows 0, lined up with the pointer.
- A drop of medicine should appear at the needle tip.
If no drop appears: check the flow again — only twice. Then change the needle and check once more. If a drop still does not appear, do not use the pen.
The reason this step is not optional is stated outright: "If no drop of medicine appears, you will not inject any medicine, even though the dose counter may move." The counter moving is not proof anything was delivered. That usually means a blocked or damaged needle.
A small drop left at the needle tip afterwards is normal and is not injected.
Setting the dose
Turn the dose selector until the counter stops at 2.4 mg. A dashed line in the counter guides you there.
"Do not set the dose by counting the number of clicks you hear." The selector clicks differently forwards and backwards, and counting is how people get it wrong — the same warning the Zepbound pen carries.
Only 2.4 mg can be selected. If the pen holds less than a full dose, the counter stops before 2.4 mg — which is the device telling you there is not enough left, not a fault.
If injecting is difficult
From the troubleshooting section: change to a firmer injection site such as the upper leg or upper arm, or try standing up while injecting into the lower stomach.
If medicine appears on the skin or squirts from the needle, the fix next time is to keep applying pressure until the yellow bar has stopped moving before lifting the pen slowly.
The pen cap rule almost nobody knows
Worth its own heading because it is genuinely surprising:
"Your pen will no longer be sterile if you store an unused pen without the cap, if you pull the pen cap off and put it on again, or if the pen cap is missing. This could lead to an infection."
Taking the cap off and putting it back is enough. So remove it only when you are ready to inject — not while gathering supplies, not to check the medicine, not out of habit.
Also: do not drop the pen or knock it against hard surfaces, do not expose it to liquids, and if you think it may be damaged, use a new one rather than trying to fix it.
Storage
- Refrigerate at 36–46°F (2–8°C), in the original carton to protect from light.
- Before the cap is removed, it may be kept at 46–86°F (8–30°C) in the original carton for up to 28 days.
- Do not freeze.
- Discard the pen if it has been frozen, exposed to light or to temperatures above 86°F, or out of the refrigerator for 28 days or longer.
Disposal, which differs from other pens
The whole Wegovy pen goes into a sharps disposal container, right away after use. The IFU says explicitly not to put it in household trash.
That is a real difference worth noting if you have used other injectables: the Zepbound pen, once its needle is removed, may go in household trash. The Wegovy pen may not, because its needle is part of it.
If you have no FDA-cleared sharps container, the IFU allows a household one that is heavy-duty plastic, closes with a tight-fitting puncture-resistant lid, stands upright and stable, is leak-resistant, and is labelled to warn of hazardous waste. Do not reuse the pen, and do not recycle the pen or the container.
Who should not inject alone
The IFU states that people who are blind or have vision problems should not use the Wegovy pen without help from someone trained to use it — the same restriction the Zepbound KwikPen carries, and for the same reason: the confirmation is visual.
It also says, twice, not to use the pen without training from your healthcare provider first.
